Terfenadine is no longer on the market in the US. Do not take terfenadine with any other medication.

GENERALLY AVOID: Fluvoxamine is an inhibitor of the CYP450 3A4 isoenzyme and may interfere with the metabolism of terfenadine and astemizole. The accumulation of terfenadine or astemizole could lead to severe or potentially fatal cardiotoxic reactions. The manufacturer recommends that fluvoxamine not be used with these drugs. However, one in vitro study suggests that fluvoxamine is a relatively weak inhibitor of the CYP450 3A4 pathway and at the usual doses is not likely to impair terfenadine clearance to a clinically significant degree.

MANAGEMENT: Caution is advised if these drugs are used together, particularly if fluvoxamine is given in high doses.
