Consumer:Using fluvoxaMINE together with ZOLMitriptan can increase the levels of ZOLMitriptan. This can cause severe headache, blurred vision, trouble concentrating, chest pain, numbness, seizure (convulsions), neck stiffness, tiredness, lack of coordination, feeling light- headed, or fainting. If your doctor prescribes these medications together, you may need a dose adjustment or special tests to safely use these medications together. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:ADJUST DOSE: The concomitant administration of CYP450 1A2 inhibitors such as fluvoxamine may increase plasma concentrations of zolmitriptan. In addition, concomitant use of serotonin reuptake inhibitors and 5-HT1 receptor agonists may potentiate the risk of serotonin syndrome, which is a rare but serious and potentially fatal condition thought to result from hyperstimulation of brainstem 5-HT1A receptors.
MANAGEMENT: It is recommended that not more than 5 mg of zolmitriptan should be taken in 24 hours during concomitant use. Close monitoring is recommended for signs and symptoms of excessive serotonergic activity such as CNS irritability, altered consciousness, confusion, myoclonus, ataxia, abdominal cramping, hyperpyrexia, shivering, pupillary dilation, diaphoresis, hypertension, and tachycardia. Particular caution is advised when increasing the dosages of these agents.
