Consumer:

Talk to your doctor before using telaprevir together with fosamprenavir. Combining these medications may decrease the blood levels of both medications, which may reduce their effectiveness in treating hepatitis C and HIV infection. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Coadministration of telaprevir and fosamprenavir/ritonavir may decrease the steady-state exposure to both telaprevir and amprenavir. The mechanism of interaction has not been described. In 18 study subjects, administration of telaprevir (750 mg every 8 hours for 10 days) in combination with fosamprenavir/ritonavir (700 mg/100 mg twice daily for 20 days) reduced the telaprevir peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin) by approximately one-third each compared to administration alone. The Cmax, AUC and Cmin of amprenavir were reduced by an average of 35%, 47% and 56%, respectively, during telaprevir coadministration. Likewise, telaprevir given in a different dosing regimen (1125 mg every 12 hours for 4 days) reduced the Cmax, AUC and Cmin of amprenavir by 40%, 49% and 58%, respectively.

MANAGEMENT: Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic drug levels, concomitant use of telaprevir and fosamprenavir/ritonavir should generally be avoided.
