Using itraconazole together with fosamprenavir may increase the effects of both medications. Contact your doctor if you experience increased side effects or if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special tests to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

ADJUST DOSE: Coadministration with amprenavir or its prodrug, fosamprenavir, may increase the plasma concentrations of itraconazole and ketoconazole, and vice versa. The mechanism is competitive inhibition of CYP450 3A4, the isoenzyme responsible for the metabolic clearance of these agents. In 12 healthy subjects, coadministration of amprenavir (1200 mg single oral dose) and ketoconazole (400 mg single oral dose) resulted in increased peak plasma concentration (Cmax) and area under the concentration-time curve (AUC) of ketoconazole by 19% and 44%, respectively, compared to administration of ketoconazole alone. Amprenavir Cmax and AUC increased by 16% and 31%, respectively. The investigators of the study did not feel these changes should be clinically important.

MANAGEMENT: Patients receiving amprenavir or fosamprenavir in combination with itraconazole or ketoconazole should be monitored for altered efficacy and safety of the drugs. Amprenavir and fosamprenavir labeling states that dosage reduction of itraconazole and ketoconazole may be required in patients receiving more than 400 mg/day of either antifungal agent. High dosages of itraconazole or ketoconazole (greater than 200 mg/day) are not recommended during treatment with the fosamprenavir-ritonavir combination regimen.
