Consumer:

Using rifAMPin together with propofol may have additive effects causing severe hypotension (excessively low blood pressure). Severe hypotension can lead to shock, collapse, coma, or even death. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment, or possibly take extra precautions for you to safely use both medications together. Avoid rising abruptly from a seated or lying-down position and contact your doctor if you experience symptoms of hypotension (low blood pressure) such as dizziness, lightheadedness, or fainting. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Coadministration with rifampin may significantly enhance the hypotensive effects of propofol. The mechanism for this interaction has not been delineated, but may involve increased production and release of endothelial nitric oxide. A retrospective case-control study involving 75 patients demonstrated that patients receiving rifampin (600 mg orally the night before and morning of surgery) and propofol (induction dose did not differ between groups) had a significantly greater reduction in mean arterial blood pressure (MAP) and duration of hypotension than propofol alone or thiopental with rifampin.

MANAGEMENT: Until more information is available, the use of rifampin with propofol should generally be avoided. Alternative drugs for induction of anesthesia should be considered. If concomitant use is required, caution and close monitoring of blood pressure are recommended. If a prolonged hypotensive episode occurs, aggressive treatment with vasopressors and fluids may be required.
