Professional:A case report suggests that methylphenidate may increase the blood concentrations of cyclosporine. The mechanism of interaction and a causal relationship have not been established. In the report, a 10-year-old heart transplant patient developed a significant increase in cyclosporine blood levels shortly after initiation of methylphenidate 5 mg twice a day. The dosage of cyclosporine was subsequently decreased and blood levels returned to therapeutic levels. Methylphenidate was eventually increased to 20 mg twice a day without further incident. Although a drug interaction was suspected by the investigators, patient noncompliance before addition of methylphenidate could not be ruled out.
