Some products containing ginkgo may increase the levels of NIFEdipine in your body. The effects of NIFEdipine may be increased. Let your doctor know if you experience dizziness, lightheadedness, flushing, fainting, or swelling in the ankle or leg while taking these medications, as they may be symptoms caused by excessive effects of NIFEdipine. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with ginkgo or ginseng has been found to increase the plasma concentrations of nifedipine in one study. The mechanism of interaction is unknown. Nifedipine is a known substrate of CYP450 3A4; however, the effects of ginkgo or ginseng on CYP450 3A4 metabolism have not been established. Limited in vitro data suggest that certain formulations such as ginkgo leaf extract or Siberian ginseng whole plant or root extracts may possess some CYP450 3A4-inhibiting activity. In the nifedipine study, pretreatment with ginkgo biloba (120 mg/day) and ginseng (200 mg/day) for 18 days increased the mean plasma concentration (at 0.5 hour postdose) of a single 10 mg dose of nifedipine by 53% and 29%, respectively, compared to administration of nifedipine alone in 22 healthy volunteers. The specific formulations of ginkgo and ginseng used were not specified.

MANAGEMENT: Patients should consult a healthcare provider before taking any herbal or alternative medicine. Nifedipine dosage may require adjustment if an interaction is suspected.
