The proposed mechanism involves inhibition by ginseng of the CYP450 3A4- and/or P-glycoprotein (P-gp)-mediated metabolism of imatinib. A case report describes a 26-year-old man with acute lobular hepatitis on biopsy. He had been taking imatinib 400 mg daily for 7 years with no complications, but over the previous 3 months, he had also been ingesting Panax ginseng daily via energy drinks. Imatinib was later restarted at the same dose with no recurrent elevations of liver enzymes. MANAGEMENT: Monitor patients closely for signs and symptoms of imatinib-induced toxicity (e.g., hepatotoxicity) during coadministration with ginseng.
