Professional:MONITOR: Coadministration with isavuconazonium sulfate (prodrug of isavuconazole) may increase the plasma concentration and effects of metformin. The proposed mechanism is inhibition of organic cation transporter 2 (OCT2)-mediated elimination by isavuconazole. Coadministration of isavuconazole sulfate with metformin increased metformin AUC by 52% and Cmax by 23%.
MANAGEMENT: Caution is advised if isavuconazonium sulfate (prodrug of isavuconazole) is used concomitantly with metformin. Dose adjustments of metformin as well as clinical and laboratory monitoring of glycemic control may be appropriate whenever isavuconazole sulfate is added to or withdrawn from therapy. Patients should be advised to be alert to potential signs and symptoms of hypoglycemia such as headache, dizziness, drowsiness, nervousness, confusion, tremor, nausea, hunger, weakness, perspiration, palpitations, and tachycardia.
