Using haloperidol together with itraconazole may increase the effects of haloperidol. Contact your doctor if you experience drowsiness, slurred speech, agitation, restlessness, convulsions, fever, an irregular heartbeat, or uncontrollable movements of the mouth, tongue, cheeks, jaw, arms or legs, or coma. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Itraconazole increases plasma concentrations of drugs metabolized by CYP450 3A4 enzymes. Itraconazole is a potent inhibitor of hepatic CYP450 3A4 enzymes. Haloperidol and reduced haloperidol have been shown to be substrates of CYP450 3A4 and to a lesser extent CYP450 2D6. Metabolic inhibition of haloperidol and reduced haloperidol may result in enhanced neurologic side effects.

MANAGEMENT: When coadministration of itraconazole and haloperidol is necessary, monitoring for adverse effects and a reduction in the haloperidol dosage should be considered. Patients should be advised to notify their physician if they experience symptoms such as extrapyramidal symptoms, drowsiness, slurred speech, agitation, restlessness, or irregular heartbeats.
