Using heparin together with nitroglycerin may decrease the effects of heparin. Call your doctor promptly if you have any signs of blood clots such as chest pain, shortness of breath, sudden loss of vision, or pain, redness or swelling in an extremity. You may need a dose adjustment in addition to special tests to safely use both medications. It is important that you tell your healthcare provider about all other medications that you are using including vitamins and herbs. Do not stop using your medications without talking to your doctor first.

MONITOR: The concurrent administration of heparin and intravenous nitroglycerin may lead to a decreased anticoagulant effect. Data are conflicting and the mechanism is unknown.

MANAGEMENT: If these agents are coadministered or if the nitroglycerin is discontinued, consider close evaluation of the coagulation status of the patient and titrate the heparin dose as needed.
