Talk to your doctor before using vardenafil together with telaprevir. Combining these medications may significantly increase the blood levels and effects of vardenafil. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if your condition changes or you experience nausea, shortness of breath, dizziness, lightheadedness, fainting, visual disturbances, ringing in the ears, vision or hearing loss, chest pain or tightness, irregular heartbeat, and/or priapism (prolonged and painful erection unrelated to sexual activity), as these may be signs and symptoms of excessive vardenafil levels. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

ADJUST DOSE: Coadministration with telaprevir may significantly increase the plasma concentrations of vardenafil. The mechanism involves inhibition of CYP450 3A4, the isoenzyme primarily responsible for the metabolic clearance of vardenafil. The interaction has not been specifically studied with telaprevir, but has been reported with other potent CYP450 3A4 inhibitors such as HIV protease inhibitors, ketoconazole, and erythromycin. According to the product labeling for vardenafil, administration of a single 5 mg dose during treatment with ritonavir 600 mg twice a day increased the vardenafil peak plasma concentration (Cmax) and systemic exposure (AUC) by 13- and 49-fold, respectively, compared to administration alone. The half-life of vardenafil, normally 4 to 5 hours, was significantly prolonged to 26 hours. Coadministration with indinavir 800 mg three times a day resulted in a 7-fold increase in the Cmax, a 16-fold increase in the AUC, and a 2-fold increase in the half-life of a single 10 mg dose of vardenafil. Similarly, coadministration of a single 5 mg dose of vardenafil with ketoconazole (200 mg once a day) resulted in a 4-fold increase in the Cmax and a 10-fold increase in the AUC of vardenafil, while coadministration with erythromycin 500 mg three times a day resulted in a 3-fold increase in the Cmax and a 4-fold increase in the AUC of vardenafil.

MANAGEMENT: Patients treated with telaprevir should not receive more than a single 2.5 mg dose of vardenafil within a 72-hour period. Patients should be advised to promptly notify their physician if they experience pain or tightness in the chest or jaw, irregular heartbeat, nausea, shortness of breath, visual disturbances, syncope, or prolonged erection (greater than 4 hours).
