Using desvenlafaxine together with indinavir may decrease the effects of indinavir. Contact your doctor if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with venlafaxine may decrease the plasma concentrations of indinavir. The mechanism of interaction has not been described. In nine healthy volunteers, administration of indinavir (800 mg single oral dose) in combination with venlafaxine (50 mg every 8 hours for 10 days) resulted in a 36% decrease in indinavir peak plasma concentration (Cmax) and a 28% decrease in systemic exposure compared to administration of indinavir alone. The clinical significance of these alterations is unknown. Indinavir had negligible effects on the pharmacokinetics of venlafaxine. Although not studied, the interaction may also be expected with desvenlafaxine, the prodrug of venlafaxine.

MANAGEMENT: Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic antiretroviral drug levels, caution is advised if indinavir is used with venlafaxine or desvenlafaxine until further information is available. Patients stabilized on their antiretroviral regimen should be monitored for potentially diminished clinical response during concomitant therapy with venlafaxine.
