Using etravirine together with methadone may decrease the effects of methadone. Contact your doctor if your condition changes or if you experience restlessness, insomnia, sweating, increased tear production, or a runny nose. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with etravirine may theoretically decrease the plasma concentrations of methadone. The proposed mechanism is etravirine induction of CYP450 3A4, the isoenzyme primarily responsible for the metabolic clearance of methadone. No significant effect of etravirine on methadone pharmacokinetics was observed in 16 study subjects receiving methadone maintenance ranging from 60 to 130 mg/day. However, an interaction may occur in susceptible patients.

MANAGEMENT: Etravirine product labeling recommends clinical monitoring for withdrawal symptoms in patients receiving methadone maintenance therapy, and the methadone dosage adjusted as necessary.
