Using etravirine together with red yeast rice may decrease the effects of red yeast rice. Contact your doctor if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with etravirine may decrease the plasma concentrations of HMG-CoA reductase inhibitors that are primarily metabolized by CYP450 3A4. The mechanism is accelerated clearance due to induction of CYP450 3A4 activity by etravirine. In 16 study subjects administered etravirine with atorvastatin 40 mg once a day, atorvastatin systemic exposure (AUC) decreased by 37%, while peak plasma concentration (Cmax) and AUC of its active metabolite 2-hydroxy-atorvastatin increased by 76% and 27%, respectively. Atorvastatin had no significant effect on the pharmacokinetics of etravirine.

MANAGEMENT: Pharmacologic response to certain HMG-CoA reductase inhibitors such as atorvastatin, lovastatin, simvastatin, and red yeast rice (which contains lovastatin) should be monitored more closely whenever etravirine is added to or withdrawn from therapy, and the dosage adjusted as necessary.
