Dolutegravir may increase the blood levels and effects of guanFACINE. You may need a dose adjustment if you have been receiving guanFACINE and are starting treatment with dolutegravir. MONITOR: Coadministration with dolutegravir may increase the plasma concentrations of drugs that are substrates of the renal organic cation transporter 2 (OCT2). Dolutegravir has been shown in vivo to inhibit tubular secretion of creatinine by inhibiting OCT2.

MANAGEMENT: Caution is advised when dolutegravir is used with drugs that are substrates of OCT2, particularly those with a narrow therapeutic range such as metformin. Dosage adjustments as well as clinical and laboratory monitoring may be appropriate for some drugs whenever dolutegravir is added to or withdrawn from therapy.
