GENERALLY AVOID: The area under the plasma concentration-time curve (AUC) of saquinavir is decreased by 62% when it is taken concomitantly with efavirenz. The mechanism is hepatic enzyme induction of CYP450 3A4 by efavirenz. Loss of antiviral activity or resistance may result. Clinically significant changes in the plasma concentration of saquinavir or efavirenz were not observed when saquinavir-ritonavir (1000 mg-100 mg twice a day) was administered with efavirenz (600 mg once daily) in healthy volunteers or HIV-infected patients. However, the combination of ritonavir-boosted saquinavir with efavirenz has been associated with an increased risk of liver toxicity.

MANAGEMENT: The concomitant use of efavirenz is not recommended if saquinavir is the only protease inhibitor in the regimen. According to product labeling, appropriate doses of the combination of saquinavir-ritonavir and efavirenz with respect to safety and efficacy have not been established. However, some authorities suggest that no dose adjustment is required, but recommend monitoring of liver function if the combination is to be used. Updated guidelines for the use of antiretroviral agents in HIV-infected patients should be consulted.
