Professional:Plasma concentrations of isoniazid may be reduced during coadministration with corticosteroids. The mechanism of interaction has not been established, but may involve enhanced hepatic and/or renal elimination of isoniazid. In a study of 26 patients with tuberculosis given isoniazid 10 mg/kg/day, investigators reported a 23% reduction in plasma isoniazid concentrations for slow acetylators and a 38% reduction for rapid acetylators following the administration of a single 20 mg dose of prednisolone. The clinical significance of this potential interaction is unknown. Prednisolone did not appear to alter the clinical response to isoniazid treatment in the study. Nonetheless, it may be advisable to monitor patient response to isoniazid more closely during concomitant corticosteroid therapy and adjust the isoniazid dosage if necessary.
