Lomitapide can increase the blood levels of warfarin and cause you to bleed more easily. You may need a dose adjustment or more frequent monitoring of your prothrombin time or INR by your doctor to safely use both medications. Call your doctor immediately if you experience any unusual bleeding or bruising, vomiting, blood in your urine or stools, headache, dizziness, or weakness during treatment with these medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration with lomitapide may increase the plasma concentrations of both enantiomers of warfarin. The mechanism of interaction has not been established. When a single 10 mg dose of warfarin was coadministered with lomitapide 60 mg once daily for 12 days, R(+) warfarin peak plasma concentration (Cmax) increased by 14% and systemic exposure (AUC) increased by 28%, while S(-) warfarin Cmax and AUC increased by 15% and 30%, respectively. Lomitapide also increased the INR by 22%. In a clinical trial of patients receiving lomitapide for homozygous familial hypercholesterolemia, one of five patients who were on concomitant warfarin therapy withdrew from the study early due to difficulty in controlling the INR.

MANAGEMENT: Caution is advised if warfarin is prescribed in combination with lomitapide. Patients should undergo regular monitoring of INR, particularly following initiation, discontinuation, or change of dosage of lomitapide. Patients should be advised to promptly report any signs of bleeding to their physician, including pain, swelling, headache, dizziness, weakness, prolonged bleeding from cuts, increased menstrual flow, vaginal bleeding, nosebleeds, bleeding of gums from brushing, unusual bleeding or bruising, red or brown urine, or red or black stools.
