A case report describes an HIV-infected patient who had been stabilized on levothyroxine for several months when he presented with an enlarged thyroid, marked lethargy, and elevated thyroid indices one month after switching from triple (zidovudine, lamivudine, saquinavir) to quadruple antiretroviral therapy (stavudine, lamivudine, ritonavir, saquinavir). The condition resolved following a doubling of the levothyroxine dosage. However, due to suspected drug-induced hepatotoxicity, indinavir was eventually substituted for ritonavir and saquinavir, which enabled a decrease of the levothyroxine dosage back to the previous level.

MANAGEMENT: During coadministration with ritonavir, the possibility of a diminished therapeutic response to levothyroxine should be considered. Dosage adjustment as well as clinical and laboratory monitoring may be appropriate whenever ritonavir is added to or withdrawn from therapy.
