Mirtazapine may reduce the effects of cloNIDine. You may need a dose adjustment or more frequent monitoring of your blood pressure if you have been taking cloNIDine and are starting treatment with mirtazapine. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: The antihypertensive effects of clonidine may be antagonized by mirtazapine. The proposed mechanism is opposing effects on adrenergic alpha-2 receptors. A case of hypertensive urgency has been reported. In addition, the risk of CNS-depressant effects may be increased.

MANAGEMENT: If concomitant use cannot be avoided, monitoring for altered efficacy of clonidine is recommended. In some cases, the dosage of clonidine may require upward adjustment.
