Coadministration with cimetidine may increase the plasma concentrations of venlafaxine but not O-desmethylvenlafaxine (ODV), its pharmacologically active metabolite. The mechanism is inhibition of hepatic first-pass metabolism of venlafaxine by cimetidine. In an open-label, crossover study consisting of 18 healthy subjects, cimetidine (800 mg orally once a day for 5 days) increased the average steady-state concentrations of venlafaxine (50 mg orally three times a day) by 61% and decreased its apparent oral clearance by 43%, with significant interpatient variability. No changes were reported in the pharmacokinetics of ODV. Because ODV is present in the circulation at much greater concentrations than those of the parent drug, the overall pharmacologic activity is expected to increase only slightly during coadministration with cimetidine. Dosage reductions should not be necessary for most patients. However, for patients with preexisting hypertension, elderly patients, and patients with hepatic dysfunction, the interaction has not been studied and may be more pronounced.
