Using rifAMPin together with lamoTRIgine may decrease the effects of lamoTRIgine. Contact your doctor if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

ADJUST DOSE: Coadministration with rifampin may decrease the serum concentrations of lamotrigine. The mechanism is rifampin induction of the hepatic glucuronidation of lamotrigine. In 10 healthy male subjects, rifampin (600 mg once a day for 5 days) increased the oral clearance (Cl/F) of a single 25 mg dose of lamotrigine by nearly 100% and decreased its terminal half-life and area under the serum concentration-time curve (AUC) by over 40% each compared to placebo. The amount of lamotrigine excreted as the glucuronide conjugate was also increased.

MANAGEMENT: An increase in the lamotrigine dosage may be required during coadministration with rifampin. If rifampin is prescribed with lamotrigine alone, a doubling of the normally recommended lamotrigine dosage may be necessary in order to achieve or maintain therapeutic levels. However, if rifampin is added to existing therapy containing lamotrigine in combination with either carbamazepine, phenytoin, phenobarbital, primidone, or valproate, then no empirical dosage adjustment is recommended based solely on the addition of rifampin. In all patients, pharmacologic response and lamotrigine serum levels should be monitored more closely following the addition or withdrawal of rifampin to guide any therapy changes. Patients should be advised to promptly notify their physician if they experience worsening of seizure control or increased adverse effects. A reevaluation of all antiepileptic agents in the regimen should be considered prior to making any changes.
