Vemurafenib may significantly increase the blood levels and effects of digoxin. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if you experience nausea, vomiting, diarrhea, loss of appetite, visual disturbances (blurred vision; light halos around objects; green or yellow vision), or an abnormally fast or slow or uneven heartbeat, as these may be signs and symptoms of excessive digoxin levels. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: Coadministration with vemurafenib may increase the serum concentrations of digoxin. The proposed mechanism is vemurafenib inhibition of the P-glycoprotein-mediated renal tubular secretion of digoxin. When a single 0.25 mg dose of digoxin was administered to 26 cancer patients treated with vemurafenib 960 mg twice daily for 22 days, digoxin peak plasma concentration (Cmax) and systemic exposure (AUC) increased by 1.5- and 1.8-fold, respectively.

MANAGEMENT: Concomitant use of digoxin with vemurafenib should generally be avoided. If coadministration is required, a dosage reduction of digoxin should be considered. Serum digoxin levels and pharmacologic effects should be closely monitored following the addition or withdrawal of vemurafenib, and the digoxin dosage adjusted as needed. Patients should be advised to seek medical attention if they experience signs of digoxin toxicity such as nausea, anorexia, visual disturbances, slow pulse, or irregular heartbeat.
