Consumer:Sodium polystyrene sulfonate may interfere with the absorption of levothyroxine and reduce its effectiveness. To minimize the interaction, sodium polystyrene sulfonate and levothyroxine should be taken at least four hours apart. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. Let your doctor know if you experience signs and symptoms of reduced thyroid function such as fatigue, weight gain, constipation, depression, or slow heart rate. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:ADJUST DOSING INTERVAL: Ion-exchange resins may bind to and decrease the gastrointestinal absorption of thyroid hormones. In one case report, a woman who had undergone a complete thyroidectomy and was receiving levothyroxine 150 mcg/day developed lethargy, hoarseness, and weight gain six months after initiation of sodium polystyrene sulfonate for chronic hyperkalemia secondary to renal failure. The symptoms resolved within six weeks after increasing the levothyroxine dosage to 200 mcg/day and separating the times of administration of the two medications by 10 hours. In an in vitro study, levothyroxine concentrations were reduced by 93% to 98% in the presence of sodium polystyrene sulfonate.
MANAGEMENT: Thyroid hormone preparations and ion-exchange resins should be administered at least 4 hours apart. Close clinical and laboratory monitoring of thyroid function is recommended during coadministration, since adjusting the dosing interval may not completely prevent the interaction.
