RifAMPin may significantly reduce the blood levels of atorvastatin, which may make the medication less effective in treating your cholesterol. The interaction may be minimized by taking both medications at the same time. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

ADJUST DOSING INTERVAL: Chronic administration of rifampin may increase or decrease the plasma concentrations of atorvastatin and atorvastatin acid depending on timing of administration. When a single 40 mg dose of atorvastatin was administered simultaneously with rifampin (600 mg once daily for 7 days), atorvastatin peak plasma concentration (Cmax) and systemic exposure (AUC) increased by 2.7-fold and 30%, respectively, due to inhibition of OATP1B1-mediated hepatic uptake by rifampin. When the dose of atorvastatin was administered 17 hours after the last dose of rifampin given for 5 days, Cmax and AUC of atorvastatin acid decreased by 40% and 80%, respectively, due to induction of CYP450 3A4-mediated metabolism by rifampin.

MANAGEMENT: To minimize impact of the interaction, manufacturers recommend that atorvastatin and rifampin doses be administered simultaneously. Monitoring for adequate hypolipidemic effect is advisable.
