Consumer:

Lofexidine may interfere with the absorption of naltrexone when they are taken orally within two hours of each other. This may reduce the blood levels and effects of naltrexone. To minimize the risk of interaction, the medications should be taken at least two hours apart. The interaction is not expected when naltrexone is given by non-oral routes such as intramuscular injection. Talk to your doctor or pharmacist if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

ADJUST DOSING INTERVAL: Coadministration with lofexidine may decrease the rate and extent of the gastrointestinal absorption of naltrexone. The mechanism of interaction has not been established. When naltrexone 50 mg/day was coadministered orally with a single 0.36 mg dose of lofexidine in 24 healthy study subjects, overall exposure was slightly reduced and the time to reach peak concentration (Tmax) was delayed by 2 to 3 hours for both naltrexone and its 6-beta-naltrexol metabolite. Naltrexone did not significantly alter the single-dose pharmacokinetics of lofexidine.

MANAGEMENT: The efficacy of oral naltrexone may be reduced if used concomitantly within 2 hours of lofexidine administration. The interaction is not expected when naltrexone is administered by non-oral routes. Administration of oral naltrexone and lofexidine should be separated by at least two hours, if possible.
