PHENobarbital may significantly reduce the blood levels of teriflunomide, which may make the medication less effective in treating your condition. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration of teriflunomide with known strong inducers of CYP450 isoenzymes and efflux transporters, such as phenobarbital and St. John's wort, may decrease the plasma concentrations and therapeutic effects of teriflunomide. The proposed mechanism is induction of CYP450 isoenzymes and/or efflux transporters responsible for the metabolic clearance of teriflunomide. Coadministration with repeated doses of rifampin (600 mg once daily for 22 days), a strong inducer of multiple CYP450 isoenzymes and efflux transporters (e.g., CYP450 2B6, 2C8, 2C9, 2C19, 3A4, BCRP, P-gp), resulted in an approximately 40% decrease in teriflunomide (70 mg single dose) exposure.

MANAGEMENT: Caution and close clinical and laboratory monitoring are recommended if teriflunomide is coadministered with strong inducers of CYP450 isoenzymes and/or efflux transporters. Dosage adjustments may be required whenever a strong inducer is added to or withdrawn from therapy.
