Relevance: 23.10.2022 Reviewer: Shkutko P.M., M.D., in

In the database of official manuals used in the service creation an interaction registered by statistical results of studies was found, which can either lead to negative consequences for the patient health or strengthen a mutual positive effect. A doctor should be consulted to address the issue of joint drug administration.

Consumer:

Using caffeine together with pimozide may increase the levels of pimozide and affect the rhythm of your heart. Contact your doctor if you experience irregular heartbeat, chest tightness, blurred vision or nausea. You may need a dose adjustment or special test if you use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration with drugs that are inhibitors of CYP450 1A2 may increase the plasma concentrations of pimozide, which is partially metabolized by the isoenzyme. While clinical data are lacking, the possibility of prolonged and/or increased pharmacologic effects of pimozide, including serious adverse effects such as tardive dyskinesia, neuroleptic malignant syndrome and QT interval prolongation, should be considered in patients receiving concomitant CYP450 1A2 inhibitors.

MANAGEMENT: Because excessive prolongation of the QT interval is associated with an increased risk of ventricular arrhythmias such as ventricular tachycardia and torsades de pointes, an ECG should be performed prior to initiating therapy with pimozide and periodically thereafter, especially during dosage adjustment or following the addition of a CYP450 1A2 inhibitor.
