Somatropin may reduce the blood levels and effects of mefloquine. Contact your doctor if your symptoms worsen or your condition changes during treatment with these medications. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with potent inducers of CYP450 3A4 may significantly decrease the plasma concentrations of mefloquine, which is primarily metabolized by the isoenzyme. In seven healthy volunteers, coadministration of mefloquine (500 mg single oral dose) with the potent CYP450 3A4 inducer rifampin (600 mg once daily for 7 days) resulted in a 19% decrease in the mean mefloquine peak plasma concentration (Cmax) and a 68% decrease in systemic exposure (AUC) compared to mefloquine administered alone. The elimination half-life of mefloquine decreased from 305 to 113 hours.

MANAGEMENT: The possibility of diminished therapeutic effects should be considered when mefloquine is used in combination with potent CYP450 3A4 inducers such as carbamazepine, dexamethasone, enzalutamide, phenobarbital, phenytoin, rifamycins, and St. John's wort. Close clinical and laboratory monitoring is recommended whenever a CYP450 3A4 inducer is added to or withdrawn from mefloquine therapy, and the dosage adjusted if necessary. Other known inducers include aminoglutethimide, barbiturates, bexarotene, bosentan, dabrafenib, efavirenz, nafcillin, nevirapine, somatrem, somatropin, and various other anticonvulsants, although the extent to which they interact with mefloquine is unknown.
