Consumer:PHENobarbital may reduce the blood levels and effects of sAXagliptin. You may need to monitor your blood sugar more closely if you have been receiving sAXagliptin and are starting treatment with PHENobarbital, and also for a week or two after you discontinue PHENobarbital. Let your doctor know if your condition changes or you experience loss of blood sugar control during treatment with these medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: Coadministration with inducers of CYP450 3A4 may decrease the plasma concentrations of saxagliptin, which is primarily metabolized by the isoenzyme. According to the product labeling, administration of a single 5 mg dose of saxagliptin in combination with the potent CYP450 3A4 inducer rifampin (600 mg once daily at steady state) resulted in decreases to saxagliptin peak plasma concentration (Cmax) by 53% and systemic exposure (AUC) by 76%. These changes were accompanied by a 39% increase in Cmax but no significant change in the AUC of the pharmacologically active metabolite, 5-hydroxysaxagliptin. Overall, the plasma dipeptidyl peptidase-4 (DPP4) inhibition activity of saxagliptin over a 24-hour dose interval was not affected by rifampin.
MANAGEMENT: Dosage adjustment of saxagliptin is not necessary during coadministration with rifampin or other CYP450 3A4 inducers. However, glycemic control should be closely monitored, and the physician notified if an interaction is suspected.
