Consumer:Using methadone together with peginterferon alfa-2b may increase the effects of methadone. Contact your doctor if you experience dizziness, drowsiness, shallow breathing, hallucinations, confusion, fast or pounding heartbeats, chest pain, trouble breathing, feeling light-headed, or faint. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: Coadministration with peginterferon alfa-2b may increase the plasma concentrations of methadone. The mechanism of interaction is unknown. In a pharmacokinetic study of 18 chronic hepatitis C patients receiving stable methadone maintenance therapy (40 mg/day or greater), treatment with peginterferon alfa-2b (1.5 mcg/kg subcutaneously once weekly) for 4 weeks was associated with a mean increase of 16% in methadone systemic exposure (AUC) compared to baseline. In 2 of the patients, however, methadone AUC approximately doubled. The clinical significance of this finding is unknown.
MANAGEMENT: Until further data are available, patients should be monitored for the signs and symptoms of increased narcotic effect during coadministration of peginterferon alfa-2b and methadone.
