Consumer:Metoclopramide may decrease the blood levels and effects of posaconazole. Contact your doctor if your condition worsens or you develop new infections during treatment with these medications. Professional:MONITOR: Coadministration with metoclopramide may decrease the plasma concentrations of posaconazole. The proposed mechanism is reduced absorption of posaconazole due to increases in gastric motility induced by metoclopramide. In 12 study subjects who received a single 400 mg dose of posaconazole oral suspension with a liquid nutritional supplement, posaconazole mean peak plasma concentration (Cmax) and systemic exposure (AUC) were reduced by approximately 20% during coadministration with metoclopramide (10 mg three times a day for 2 days). The clinical significance is unknown, but reduced antifungal efficacy is possible.
MANAGEMENT: Caution is advised if posaconazole is prescribed in combination with metoclopramide. Patients should be monitored for the development of breakthrough fungal infections.
