Professional:The combination of ciprofloxacin and metronidazole has been reported to increase the serum concentration of quinidine in a patient treated for atrial fibrillation and suspected diverticulitis. The patient's trough serum quinidine level was 6.3 mcg/mL during coadministration with the antibiotics and declined to 2.3 mcg/mL 3 days after completion of the antibiotics. No clinically significant adverse effects were observed. The exact mechanism of interaction is unknown. Neither metronidazole nor ciprofloxacin are known to significantly inhibit CYP450 3A4, the isoenzyme responsible for the metabolism of quinidine. Moreover, ciprofloxacin (750 mg orally every 12 hours for 5 days) was found to have no significant effect on the pharmacokinetics or pharmacologic effects of quinidine (400 mg single oral dose) in a study of seven healthy volunteers.
