Using rifabutin together with tipranavir may increase the effects of rifabutin. Contact your doctor if you experience nausea, vomiting, tiredness, abdominal pain, yellow skin or eyes, and unconsciousness. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

ADJUST DOSE: Coadministration with tipranavir in combination with low-dose ritonavir may significantly increase the plasma concentrations of rifabutin and its active 25-O-desacetyl metabolite. The mechanism is inhibition of rifabutin metabolism via CYP450 3A4. In 20 study subjects, tipranavir/ritonavir (500 mg/200 mg twice a day for 7 days) increased the mean peak plasma concentration (Cmax), area under the concentration-time curve (AUC) and trough plasma concentration (Cmin) of rifabutin (150 mg single dose) by 70%, 190% and 114%, respectively, compared to administration of rifabutin alone. The Cmax, AUC and Cmin of the 25-O-desacetyl metabolite were approximately 3, 20, and 8 times the values observed with rifabutin alone. Rifabutin had negligible effects on the pharmacokinetics of tipranavir.

MANAGEMENT: During coadministration with tipranavir/ritonavir, a reduction in the rifabutin dosage of up to 75% is recommended (e.g., 150 mg every other day or three times per week). Patients should be monitored for rifabutin toxicity such as leukopenia, uveitis, arthralgias and skin discoloration. Further dosage reduction may be necessary.
