Erythromycin can significantly increase the blood levels of naloxegol. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. GENERALLY AVOID: Coadministration with moderate inhibitors of CYP450 3A4 may significantly increase the plasma concentrations of naloxegol, which is primarily metabolized by the isoenzyme. When a single 25 mg dose of naloxegol was administered during multiple dosing of 240 mg once daily extended-release diltiazem, a moderate CYP450 3A4 inhibitor, naloxegol peak plasma concentration (Cmax) and systemic exposure (AUC) increased by approximately 2.9- and 3.4-fold, respectively. Increased exposure to naloxegol may precipitate opioid withdrawal symptoms such as hyperhidrosis, lacrimation, rhinorrhea, chills, diarrhea, abdominal pain, anxiety, insomnia, irritability, restlessness, and yawning.

MANAGEMENT: Concomitant use of naloxegol with moderate CYP450 3A4 inhibitors should generally be avoided. If coadministration is required, the dosage of naloxegol should be reduced to 12.5 mg once daily and the patient closely monitored for opioid withdrawal symptoms.
