Using cycloSPORINE together with niCARdipine can cause damage to the kidneys or cause nerve loss. Tell your doctor if you experience dizziness, numbness, skin tingling, muscle twitching, or seizures which may be signs of nerve damage. Talk with your doctor if you develop fever, chills, body aches, flu symptoms, and severe pain in your upper stomach spreading to your back. If you take both medications together, tell your doctor if you have any of these symptoms. You may need a dose adjustment or special test if you use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Nicardipine may increase blood cyclosporine levels. One study of 21 renal transplant recipients demonstrated a 19% to 214% increase in cyclosporine levels in patients concomitantly receiving oral nicardipine. The mechanism is not known, but may be decreased hepatic metabolism of cyclosporine. The risk of cyclosporine-induced nephrotoxicity and neurotoxicity may be increased.

MANAGEMENT: Cyclosporine levels and renal function, including serum creatinine, should be carefully monitored. The dosage should be adjusted as needed. Patients should be advised to notify their physician if they experience nausea, vomiting, diarrhea, abdominal pain, dizziness, fatigue, or headache. Amlodipine, isradipine and nitrendipine are not expected to affect cyclosporine to the same extent and may be considered as alternatives.
