Using cycloSPORINE together with sertraline may cause increased side effects and could cause a condition know as serotonin syndrome. Contact your doctor if you experience dizziness, drowsiness, nausea, vomiting, rapid heartbeat, agitation, tremor, confusion, seizures, and others. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: The concomitant administration of sertraline and cyclosporine been associated with serotonin syndrome. The mechanism is unknown but may involve inhibition of CYP450 3A4 metabolism by cyclosporine. In a single case report, a 53-year-old renal transplant patient taking cyclosporine 100 mg twice daily developed symptoms of serotonin syndrome 5 days after sertraline 50 mg/day was added to his regimen. In addition, the combination has been associated with increased cyclosporine blood concentrations, although other concomitant drugs may have confounded the results. The mechanism may involve competition for CYP450 3A4-mediated metabolism, of which both drugs are substrates.

MANAGEMENT: Until more information is available, monitoring patients for adverse effects, and changes in cyclosporine concentrations and renal function may be advisable. Patients should be advised to notify their physician if they experience symptoms of cyclosporine toxicity (including nausea, vomiting, diarrhea, abdominal pain, dizziness, fatigue, or headache) or signs of excessive serotonergic activity (CNS irritability, altered consciousness, confusion, myoclonus, ataxia, abdominal cramping, hyperpyrexia, shivering, pupillary dilation, diaphoresis, hypertension, and tachycardia).
