Using cyclophosphamide together with tamoxifen may increase the risk of dangerous blood clots, which can lead to serious conditions such as stroke, heart attack, heart failure, collapse, and kidney failure. You should seek immediate medical attention if you experience potential signs and symptoms of blood clots such as chest pain, shortness of breath, difficulty breathing, coughing up blood, sudden loss of vision, and/or pain, redness or swelling in an arm or leg. Talk to your doctor or pharmacist if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration of tamoxifen with cyclophosphamide may increase the risk of thromboembolic complications. The mechanism of this interaction has not been delineated.

MANAGEMENT: Patients receiving tamoxifen with cyclophosphamide should receive clinical and laboratory monitoring for the development of thromboembolic events. It is not known whether prophylactic anticoagulation or antiplatelet therapy may lessen the risk of thromboembolic events; however, some authorities suggest that prophylactic antithrombotic measures should be considered if cyclophosphamide is given with tamoxifen. Patients should be advised to seek medical attention if they develop potential signs and symptoms of thromboembolism such as chest pain, shortness of breath, and pain or swelling in the arms or legs.
