Ask your doctor before using ketoconazole together with nevirapine. Using these medications together may decrease the effects of ketoconazole and increase the levels of nevirapine. Contact your doctor if your condition or infection worsens. If your doctor prescribes these medications together, you may need a dose adjustment or special tests to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: The concomitant use of ketoconazole and nevirapine may significantly decrease plasma concentrations of ketoconazole and increase concentrations of nevirapine. When ketoconazole 400 mg/day was coadministered with nevirapine 200 mg/day for 14 days, followed by 400 mg/day for 14 days, the area under the concentration-time curve (AUC) of ketoconazole decreased by an average of 72% and the maximum plasma concentration (Cmax) decreased by 44%. Nevirapine plasma AUCs increased by 15% to 28%, which is not considered clinically significant. Metabolic induction of CYP450 3A4 by nevirapine and inhibition of CYP450 3A4 by ketoconazole may be involved. The antifungal efficacy of ketoconazole may be reduced.

MANAGEMENT: Coadministration should generally be avoided.
