Using sirolimus together with posaconazole is not recommended. Combining these medications may increase the blood levels of sirolimus to dangerous levels. CONTRAINDICATED: Coadministration with posaconazole may significantly increase the plasma concentrations of sirolimus. The proposed mechanism is posaconazole inhibition of sirolimus metabolism via intestinal and hepatic CYP450 3A4. In healthy volunteers, administration of a single 2 mg oral dose of sirolimus in combination with posaconazole suspension (400 mg twice a day for 16 days) increased mean sirolimus peak plasma concentration (Cmax) and systemic exposure (AUC) by 6.7- and 8.9-fold, respectively. Sirolimus toxicity may occur.

MANAGEMENT: Concomitant use of sirolimus with posaconazole is considered contraindicated.
