Using sirolimus together with posaconazole is not recommended. Combining these medications may increase the blood levels of sirolimus to dangerous levels. You may be more likely to experience serious side effects such as infections, certain types of cancer, fluid retention, swelling, high cholesterol or triglycerides, and kidney problems. Talk to your doctor if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

CONTRAINDICATED: Coadministration with posaconazole may significantly increase the plasma concentrations of sirolimus. The proposed mechanism is posaconazole inhibition of sirolimus metabolism via intestinal and hepatic CYP450 3A4. In healthy volunteers, administration of a single 2 mg oral dose of sirolimus in combination with posaconazole suspension (400 mg twice a day for 16 days) increased mean sirolimus peak plasma concentration (Cmax) and systemic exposure (AUC) by 6.7- and 8.9-fold, respectively. Sirolimus toxicity may occur.

MANAGEMENT: Concomitant use of sirolimus with posaconazole is considered contraindicated.
