Probenecid may increase the blood levels of baricitinib. This may increase the risk of side effects such as infections, blood clots, anemia, increased cholesterol and lipid levels, liver problems, and certain types of cancers. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Coadministration with potent inhibitors of organic anion transporter 3 (OAT3) may increase the plasma concentrations and the risk of adverse effects of baricitinib, which has been shown in vitro to be a substrate of the transporter. In a clinical study, administration with probenecid, a potent OAT3 inhibitor, resulted in a 2-fold increase in baricitinib systemic exposure (AUC) with no change in Cmax or Tmax. Simulations with diclofenac and ibuprofen (less potent OAT3 inhibitors) predicted minimal effect on the pharmacokinetics of baricitinib.

MANAGEMENT: Concomitant use of baricitinib with potent OAT3 inhibitors is not recommended. Patients treated with baricitinib should be monitored for the development of adverse effects such as infections, malignancies, thrombosis, hematologic abnormalities, gastrointestinal perforations, hyperlipidemia,, and hepatic transaminase elevations.
