Simeprevir may increase the blood levels and effects of vardenafil in some patients. Contact your doctor if your condition changes or you experience nausea, shortness of breath, dizziness, lightheadedness, fainting, visual disturbances, ringing in the ears, vision or hearing loss, chest pain or tightness, irregular heartbeat, and/or priapism (prolonged and painful erection unrelated to sexual activity), as these may be signs and symptoms of excessive vardenafil levels. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with simeprevir may result in mild increases in plasma concentrations of phosphodiesterase-5 (PDE5) inhibitors, which are primarily metabolized by CYP450 3A4. In vivo, simeprevir mildly inhibits intestinal CYP450 3A4, but does not affect hepatic CYP450 3A4 activity. Therefore, a major pharmacokinetic interaction is not expected under normal circumstances.

MANAGEMENT: When prescribed with simeprevir, no empiric dosage adjustment is required for PDE5 inhibitors in the treatment of erectile dysfunction. However, dosage adjustment of sildenafil or tadalafil may be needed when these agents are administered chronically for the treatment of pulmonary arterial hypertension. Consider starting with the lowest effective dose and increase as needed, with clinical monitoring as appropriate.
