Consumer:

Paritaprevir may significantly increase the blood levels of rilpivirine. This may increase the risk and/or severity of side effects such as depression, liver problems, and irregular heart rhythm. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Coadministration with ombitasvir/paritaprevir/ritonavir plus dasabuvir may significantly increase the plasma concentrations of rilpivirine. The mechanism of interaction has not been established, but may involve inhibition of CYP450 3A4-mediated metabolism by ritonavir and paritaprevir. In 10 study subjects, administration of rilpivirine 25 mg once daily in the morning in combination with once daily morning doses of ombitasvir/paritaprevir/ritonavir (25 mg/150 mg/100 mg) plus twice daily doses of dasabuvir resulted in increases of approximately 2.6.-, 3.3- and 3.6-fold in mean rilpivirine peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin), respectively. Similar increases were observed when rilpivirine was dosed in the evening with food or 4 hours after food. High plasma levels of rilpivirine may increase the risk of QT interval prolongation, which has been associated with ventricular arrhythmias including torsade de pointes and sudden death. Rilpivirine had minor to no effects on the pharmacokinetics of ombitasvir, paritaprevir, ritonavir, and dasabuvir.

MANAGEMENT: Concomitant use of rilpivirine with ombitasvir/paritaprevir/ritonavir plus dasabuvir is not recommended.
