Voriconazole may increase the blood levels and effects of cloBAZam. This may increase the risk and/or severity of side effects such as drowsiness, tiredness, drooling, constipation, and breathing difficulties. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Avoid driving or operating hazardous machinery until you know how the medications affect you, and use caution when getting up from a sitting or lying position. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration of clobazam with inhibitors of CYP450 2C19 may significantly increase the plasma concentrations of its active metabolite, N-desmethylclobazam, which is primarily metabolized by the isoenzyme. Based on extrapolation from pharmacogenomic data, inhibition of CYP450 2C19 may result in up to a 5-fold increase in exposure to N-desmethylclobazam. At therapeutic dosages of clobazam, plasma concentrations of N-desmethylclobazam are 3 to 5 times higher than those of the parent compound. Based on animal and in vitro receptor binding data, estimates of the relative potency of N-desmethylclobazam compared to parent compound range from 1/5 to equal potency.

MANAGEMENT: Dosage adjustment of clobazam may be necessary when coadministered with strong (e.g., fluconazole, fluvoxamine, ticlopidine) or moderate (e.g., omeprazole) inhibitors of CYP450 2C19.
