Voriconazole may increase the blood levels and effects of cloBAZam. MONITOR: Coadministration of clobazam with inhibitors of CYP450 2C19 may significantly increase the plasma concentrations of its active metabolite, N-desmethylclobazam, which is primarily metabolized by the isoenzyme. Based on extrapolation from pharmacogenomic data, inhibition of CYP450 2C19 may result in up to a 5-fold increase in exposure to N-desmethylclobazam. At therapeutic dosages of clobazam, plasma concentrations of N-desmethylclobazam are 3 to 5 times higher than those of the parent compound. Based on animal and in vitro receptor binding data, estimates of the relative potency of N-desmethylclobazam compared to parent compound range from 1/5 to equal potency.

MANAGEMENT: Dosage adjustment of clobazam may be necessary when coadministered with strong (e.g., fluconazole, fluvoxamine, ticlopidine) or moderate (e.g., omeprazole) inhibitors of CYP450 2C19.
