Consumer:Before using doxycycline, tell your doctor if you also use phenytoin. You may need a dose adjustment or special tests in order to safely take both medications together. This may decrease the effectiveness of doxycycline. If you take both medications together, tell your doctor if your infection worsens. Professional:MONITOR: Phenytoin (and other hydantoins) may significantly decrease doxycycline half-life resulting in decreased antimicrobial effectiveness. The mechanism may be related to induction of doxycycline metabolism.
MANAGEMENT: Patients should be monitored for altered doxycycline efficacy. It may be necessary to increase doxycycline dosage. Alternative antimicrobial therapy with a different tetracycline or a different class of antimicrobial agent may be preferable.
