The proposed mechanism is decreased clearance due to pexidartinib-mediated inhibition of P-gp. Concomitant administration of a single oral dose of pexidartinib (1800 mg) increased the peak plasma concentration (Cmax) and systemic exposure (AUC) of digoxin, a P-gp substrate, by 30% and 9%, respectively, compared with digoxin alone. MANAGEMENT: Caution is advised if pexidartinib is used concomitantly with drugs that are substrates of P-gp, particularly those with a narrow therapeutic range. The potential for diminished therapeutic effects should be considered when pexidartinib is prescribed in combination with drugs that are P-gp substrates. Dosage adjustments as well as clinical and laboratory monitoring should be considered whenever pexidartinib is added to or withdrawn from therapy with these drugs.
