Consumer:Using phenytoin together with trimethoprim may increase the effects of phenytoin. Contact your doctor if you experience twitching eye movements, slurred speech, loss of balance, tremor, muscle stiffness or weakness, nausea, vomiting, feeling light-headed, fainting, and slow or shallow breathing. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: Trimethoprim may inhibit the CYP450 hepatic metabolism of hydantoins. Serum hydantoin levels and risk of toxicity may be increased. Data are available for phenytoin.
MANAGEMENT: Monitoring for clinical and laboratory evidence of hydantoin toxicity is recommended. Dose adjustments may be necessary. Patients should be advised to notify their physician if they experience symptoms of hydantoin toxicity (e.g., drowsiness, visual disturbances, change in mental status, seizures, nausea, or ataxia).
