Using pimozide together with sertraline is not recommended. Combining these medications can increase the blood levels of pimozide to dangerous levels and cause an irregular heart rhythm that may be serious and potentially life-threatening. You should seek immediate medical attention if you develop sudden dizziness, lightheadedness, fainting, shortness of breath, or heart palpitations during treatment with pimozide. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

CONTRAINDICATED: Coadministration with sertraline may increase the plasma concentrations of pimozide. The mechanism of interaction has not been established. In 15 healthy volunteers, administration of a single 2 mg dose of pimozide during sertraline treatment 200 mg/day resulted in 35% and 37% increases in mean pimozide peak plasma concentration (Cmax) and systemic exposure (AUC), respectively, compared to administration of pimozide alone. While these increases were not associated with significant prolongation of the QTc interval (>=15% from baseline) in the study subjects, the effect of sertraline on pimozide given at dosages greater than a single 2 mg dose is not currently known. The use of pimozide has been associated with dose-related prolongation of the QT interval, thus elevated plasma levels may potentiate the risk of ventricular arrhythmias such as ventricular tachycardia and torsade de pointes as well as cardiac arrest and sudden death. Moreover, sertraline itself has been associated with rare cases of QT prolongation and torsade de pointes during postmarketing use, thus an additive pharmacodynamic effect with pimozide may occur as well.

MANAGEMENT: Given the narrow therapeutic index of pimozide and the potential for interaction with sertraline, concomitant administration of these agents is considered contraindicated.
