Professional:ADJUST DOSE: Posaconazole may significantly increase cyclosporine concentrations by inhibiting CYP450 3A4 metabolism. The risk of cyclosporine-related adverse effects may be increased. Serious adverse events including nephrotoxicity, leukoencephalopathy, and death were reported during clinical studies. Cyclosporine dosage reductions of up to 29% were required. Conversely, cyclosporine may increase plasma levels of posaconazole by inhibiting its clearance via P-glycoprotein-mediated efflux.
MANAGEMENT: Cycloporine dosage should be reduced to approximately 75% of the original dosage upon initiation of posaconazole treatment. Cyclosporine blood levels and renal function, including serum creatinine, should be carefully monitored during concomitant use and upon withdrawal of posaconazole. Patients should be advised to notify their physician if they experience symptoms that could indicate cyclosporine toxicity such as nausea, vomiting, diarrhea, abdominal pain, dizziness, fatigue, or headache.
